Abode Care Partners is expanding its Norfolk team and seeking a compassionate Nurse Practitioner to deliver primary care in patients' homes. A $10,000 sign-on bonus is offered, with a compensation structure designed to reward expertise and dedication through quarterly incentives and increased annual totals.

Responsibilities include clinical assessments, personalized care plans in skilled nursing and independent living settings, and collaboration with interdisciplinary teams to improve outcomes

Formación

• Active state NP or PA license and CPR certification.
• MSN required for NP; PA may not require a masters.
• Current state license and national certification for NP/PA.
• DEA licensure in good standing.
• 5+ years nursing healthcare experience; 2+ years NP/PA preferred.
• Strong communication and time management abilities.

Responsabilidades

• Provide evidence-based medical care and develop care plans.
• Treat chronic and acute conditions with home-based primary care.
• Order/interpret labs and diagnostics as needed.
• Collaborate with interdisciplinary teams for care transitions.
• Refer to specialists when required and maintain records.
• Manage patient records and ensure HIPAA compliance.
