Job summary

Abode Care Partners seeks a compassionate Nurse Practitioner to deliver comprehensive primary care to seniors and individuals with complex medical needs in their homes and residences. You will conduct clinical assessments, create personalized care plans, and work with interdisciplinary teams to ensure high-quality transitions.

Qualifications

• Active state NP license required.
• Current Nurse Practitioner National Board Certification.
• Active state DEA licensure.
• MSN or higher in Nursing.
• Five+ years nursing experience; NP experience preferred.

Responsibilities

• Provides primary care to seniors and patients with complex health needs in home/residence settings.
• Conducts clinical assessments and develops personalized care plans.
• Collaborates with interdisciplinary teams to ensure seamless care transitions.
• Manages chronic conditions and acute visits with emphasis on preventative care.
• Maintains up-to-date patient medical records and documentation.
