Job summary

Abode Care Partners, an affiliate of BrightSpring Health Services, seeks a compassionate Nurse Practitioner to join our Richmond, VA team providing comprehensive primary care in patients' homes, including skilled nursing, assisted living, and independent living communities.

Qualifications

• Active state license as NP/PA and current CPR
• MSN required for NP; PA path allows RN-based credentialing
• Maintain current state license and national certification for NP/PA
• If NP, must be licensed as a Registered Nurse in good standing in state employed
• Five or more years of nursing healthcare experience; 2+ years NP/PA preferred
• Excellent interpersonal and time management skills
• Proficient with medical instruments and HIPAA/OSHA regulations
• Experience with EMR and Microsoft Office applications

Responsibilities

• Provides comprehensive primary care in home-based settings
• Performs clinical assessments and develops personalized care plans
• Orders and interprets laboratory and diagnostic tests
• Collaborates with interdisciplinary teams for seamless care
• Issues referrals to other healthcare practitioners when needed
• Maintains up-to-date patient medical records
