Abode Care Partners in Henrico, VA is seeking a compassionate Nurse Practitioner to join our home-based primary care team. In this role you will conduct clinical assessments, develop personalized care plans, and deliver comprehensive care in seniors' homes across skilled nursing, assisted living and independent living communities.

You will work with an interdisciplinary team to improve patient outcomes, manage chronic conditions, and participate in a quarterly incentive program, with a $10,000

Qualifications

• Active state NP/PA license and CPR certification.
• MSN for NP; PA practice without MSN per role.

Responsibilities
