Job summary

Scylla Solutions LLC seeks a Licensed Clinical Social Worker to join our integrated clinical team onsite at long-term care facilities. You will provide therapy, assessments, care planning, and behavioral health support with a Monday–Friday schedule and autonomy in your daily workflow.

This role offers strong administrative and clinical support, interdisciplinary collaboration, HIPAA-compliant systems, and opportunities for geriatric-focused training as you help residents thrive.

Qualifications

• Active, unrestricted LCSW license.
• Valid driver's license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Experience in geriatric or long-term care settings.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances.
• Deliver behavioral management and brief, goal-oriented interventions aligned with geriatric best practices.
• Work with nursing, social services, rehabilitation and medical teams to coordinate care.
• Maintain complete and timely documentation using GNT and adhere to HIPAA guidelines.
