Job summary

Scylla Solutions LLC in Illinois seeks a Licensed Clinical Social Worker (LCSW) to join our integrated clinical team in long-term care facilities. Provide therapy, assessments, care planning, and behavioral health support onsite with a Monday–Friday schedule and strong clinical and administrative support.

You will collaborate with nursing and medical staff, document in the GNT system, uphold HIPAA standards, and help residents live well.

Qualifications

• Active, unrestricted LCSW license.
• MSW degree from an accredited program.
• Ability to document accurately and timely in a geriatric setting.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, and behavioral issues.
• Deliver behavioral management and brief, goal-oriented interventions.
• Collaborate with nursing, social services, rehabilitation, and medical staff to coordinate care.
• Participate in care plan discussions, facility meetings, and case collaborations.
• Maintain complete documentation using the GNT system and ensure HIPAA-compliant workflows.
