Job summary

FOX Rehabilitation in White Oak, MD is seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences or senior living communities. You’ll design personalized plans and provide evidence-based therapy, with the flexibility to set your own schedule and approach.

The Geriatric House Calls model may include telehealth options, enabling continued care across a wider patient base while maintaining autonomy and meaningful outcomes for patients and families.

Qualifications

• Licensed PT in Maryland or eligible to obtain license.
• Degree from an accredited Physical Therapy program.
• CPR certification and EMR proficiency required.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document accurately in FOX’s EMR in line with regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
