FOX Rehabilitation in Reston, VA is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients’ homes or Senior Living Communities under the Geriatric House Calls model. Qualifications

• Licensed Physical Therapist required (state license or eligible)
• DPT/Physical Therapy degree from accredited program
• Basic computer and EMR proficiency
• Current CPR certification required

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living communities
• Perform evaluations and develop individualized plans of care
• Deliver therapy sessions to improve mobility and independence
• Document in EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Provide telehealth/virtual care when appropriate
