Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences or senior living communities through our Geriatric House Calls model. You’ll tailor treatment plans, evaluate progress, and schedule flexible in-home visits with autonomy.

Clinicians may incorporate telehealth, collaborate with an interdisciplinary team, and grow professionally in a supportive, clinician-led culture that values independence, mobility, and aging in place.

Qualifications

• Valid Physical Therapy license in the state of practice (NY).
• Degree from an accredited Physical Therapy program (DPT preferred).
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
