Job summary

West Coast Wound is seeking a skilled Physical Therapist to provide in-home care across the Hemet/Temecula region. You will perform full home evaluations, develop plans of care, and lead patients through personalized rehab programs.

The role requires CA licensure, strong clinical reasoning, and the ability to work independently with a mobile patient caseload. Four 10-hour days per week in a home-based setting.

Qualifications

• CA license in good standing is required.
• Graduation from an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, and rehab principles.
• Strong clinical reasoning and patient assessment skills.
• Ability to independently perform comprehensive PT evaluations and create plans of care.
• Experience with orthopedic, musculoskeletal, neuro, balance, gait, and functional limits preferred.
• Excellent communication and ability to work in patients' homes.

Responsibilities

• Travel to patients' homes to provide physical therapy services.
• Perform comprehensive home PT evaluations and develop plans of care.
• Establish measurable short- and long-term functional goals.
• Provide therapeutic exercises, gait and mobility training, balance work, and manual therapy when appropriate.
• Instruct patients on home exercise programs and use of assistive devices.
• Educate patients and caregivers on diagnosis, treatment, safety, and independence.
• Monitor response to treatment and modify the plan as needed.
• Document evaluations, treatment notes, progress, and discharge summaries.
• Coordinate care with physicians, nurses, and other therapists.
• Maintain confidentiality and professional boundaries in home visits.
