Job summary

You will perform admissions, care plan updates, status assessments, and patient education, while collaborating with the interdisciplinary team.

Qualifications

• Minimum 1 year RN experience.
• Home health and OASIS experience preferred.
• Strong assessment, communication, and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Perform admissions, resumptions of care, recertifications, and discharge visits.
• Complete OASIS assessments and maintain accurate clinical documentation.
• Develop and update individualized care plans based on physician orders.
• Assess patient status, monitor progress, and communicate changes to the care team.
• Educate patients and caregivers on medications, disease management, and home care techniques.
• Coordinate services with physicians and interdisciplinary team members.
• Supervise and support LVNs and HHAs as needed.
• Participate in case conferences and ongoing clinical education.
