Job summary

Qualifications

Responsibilities

• Perform admissions, resumptions of care, recertifications, and discharge visits.
• Complete OASIS assessments and maintain accurate clinical documentation.
• Develop and update individualized care plans based on physician orders.
• Assess patient status, monitor progress, and communicate changes to the care team.
• Educate patients and caregivers on medications, disease management, and home care techniques.
• Coordinate services with physicians and interdisciplinary team members.
• Supervise and support LVNs and HHAs as needed.
• Participate in case conferences and ongoing clinical education.

Location:

Open from Redwood city to San Jose, South Bay Area. Flexible territory assignments available, with patient visits coordinated around your preferred coverage area and proximity to home whenever possible.

Pay:

$55.00–$75.00 per hour DOE
