Job summary

OSF HealthCare is seeking an RN Home Health professional to collaborate with the care team and implement individualized home health plans of care. You will coordinate with nursing, therapy, social work and home infusion services to ensure optimal patient outcomes and adherence to OASIS documentation standards.

The role requires nursing program completion and an active RN license in IL or MI, with BLS certification and strong communication skills.

Qualifications

• Nursing program completion is required.
• RN licensure in IL or MI depending on location.
• Current AHA HealthCare Provider BLS before start date.
