Qualifications

• Must hold valid RN license in Massachusetts and NH.
• 3–5 years of nursing experience with home health and OASIS preferred.
• Self-directed with minimal supervision and strong planning skills.
• Excellent communication with physicians and care teams.

Responsibilities

• Coordinate patient care for a caseload, delegating to LPNs and monitoring visits.
• Complete initial assessment, develop and revise the plan of care with goals.
• Administer medications and provide patient/family education.
• Document in EHR and ensure OASIS completion per policy.
• Participate in weekend/holiday/on-call rotations.
