Qualifications

• Graduate from an accredited school of nursing; licensure and current registration as a Registered Professional Nurse in NY required.
• BSN preferred; BLS Certification preferred; OASIS Certification preferred.

Responsibilities

• Coordinate all services provided to the patient by the agency, informal supports, and community resources, and review the plan of care as needed including plan for discharge and implementation of discharge plan.
• Demonstrate familiarity with insurance reimbursement and communicate this to patients, families, caregivers and all providers.
• Communicate information to patients, families and caregivers verbally and in writing; function autonomously and make decisions as part of the home care team.
