Job summary

Kona Medical Consulting and The Home Health Team seek a Home Health Field Registered Nurse (RN) to deliver skilled nursing care in patients' homes across the Dallas–Fort Worth Metroplex. The role emphasizes reliability, clinical judgment, and patient-centered care.

Responsibilities include in-home assessments, care plan development, wound care, education, and coordination with physicians and care teams. On-call involvement and compassionate, professional service are expected.

Qualifications

• Texas RN license required.
• 1+ year nursing experience; Home Health preferred but not required.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• In-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medications, and patient education.
• Communicate with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation per regulatory requirements.
• Participate in on-call rotation as needed; build rapport with patients and families.
