Job summary

PurposeCare is seeking a Registered Nurse (RN) for a full-time Home Health position in the Midwest region. You will manage a patient caseload in homes, collaborating with physicians, therapists, and an interdisciplinary team to deliver high-quality, patient-centered care.

The role emphasizes autonomy, flexible scheduling, and the opportunity to build lasting relationships with patients and families while adhering to Medicare regulations and agency standards.

Qualifications

• Active RN license in good standing and eligible to practice.
• Minimum of 1 year Home Health experience.
• Knowledge of Medicare regulations and OASIS documentation.
• Reliable transportation and valid driver’s license.

Responsibilities

• Complete Start of Care, Resumption of Care, and Recertification assessments.
• Develop individualized care plans supporting patient goals and independence.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Educate patients and families on disease management, medications, and safety.
• Complete timely documentation and OASIS assessments.
• Monitor patient progress and adjust care plans as needed.
• Supervise Home Health Aides and collaborate with LPNs.
• Participate in case conferences and interdisciplinary care planning.
• Help ensure compliance with Medicare Conditions of Participation and agency standards.
