Job summary

PurposeCare is seeking an experienced Registered Nurse (RN) for Home Health to join our Midwest team in Illinois. This full-time role offers flexible scheduling, autonomy, and the chance to provide one-on-one patient care in the home setting.

You will assess, plan, and coordinate care, Start of Care and Recertifications, educate patients and families, and collaborate with interdisciplinary teams to improve outcomes while adhering to Medicare guidelines.

Qualifications

• Active Registered Nurse (RN) license required.
• At least 1 year of Home Health experience preferred.
• Strong knowledge of OASIS documentation and Medicare regulations.
• Excellent assessment and clinical decision‑making skills.
• Effective communication and relationship-building abilities.

Responsibilities

• Oversee caseload within assigned territory and collaborate with physicians, therapists, and interdisciplinary team members.
• Complete Start of Care, Resumption of Care, and Recertification assessments.
• Develop individualized care plans aligned with patient goals and independence.
• Educate patients and families on disease management, medications, and safety.
• Document accurately and perform OASIS assessments in a timely manner.
• Monitor patient progress and adjust care plans as needed.
• Supervise Home Health Aides and coordinate with LPNs as appropriate.
• Participate in case conferences and care planning to improve outcomes.
• Ensure compliance with Medicare Conditions of Participation and agency standards.
