Job summary

PurposeCare is seeking an experienced Registered Nurse (RN) for full-time Home Health in Illinois. You will manage a patient caseload in the home, coordinating with physicians, therapists, and the care team to deliver compassionate, high-quality care.

The ideal candidate has an active RN license and at least one year of Home Health experience, with strong assessment and clinical decision-making skills. You will educate patients and families, complete Start of Care and Recertification

Qualifications

• Active RN license in the state
• Minimum of one year of Home Health experience
• Strong knowledge of OASIS documentation and Medicare regulations
• Excellent assessment and clinical decision-making skills
• Outstanding communication and relationship-building abilities
• Valid driver's license with reliable transportation

Responsibilities

• Oversee a caseload of patients in assigned territory and collaborate with physicians, therapists, and interdisciplinary team members
• Complete Start of Care, Resumption of Care, and Recertification assessments
• Develop and implement individualized care plans supporting patient goals
• Coordinate care with physicians, therapists, social workers, and caregivers
• Educate patients and families on disease management, medications, and safety
• Complete timely documentation and OASIS assessments
• Monitor patient progress and adjust care plans as needed
• Supervise Home Health Aides and collaborate with LPNs as appropriate
• Participate in case conferences and interdisciplinary care planning
• Ensure compliance with Medicare Conditions of Participation and agency quality standards
