VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will evaluate mobility, develop personalized plans, and provide hands-on therapy, collaborating with physicians and the care team to optimize outcomes.

You'll supervise PTAs where allowed, educate patients and caregivers, and participate in care conferences. A DPT or Master’s, state license, CPR, and a valid driver’s license are required.

Qualifications

• Graduation from an accredited Physical Therapy program (DPT or Master’s) and current PT license in the practicing state.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification and valid driver’s license with reliable transportation.

Responsabilités

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately within the EMR.
• Participate in case conferences and team meetings.
