VitalCaring is seeking a motivated Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will evaluate mobility, develop and implement personalized plans, and supervise PTAs while collaborating with an interdisciplinary team.

Formación

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.

Responsabilidades

• Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately in the EMR.
• Collaborate with nurses, PTAs, and physicians to ensure coordinated care.
• Supervise PTAs in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
