Job summary

Optum is seeking a Home Health Physical Therapist to provide in-home care across Sullivan County. The role offers a full-time, flexible schedule with a focus on autonomy and one-on-one patient care.

You will evaluate functional status, develop and execute treatment plans, and coordinate with physicians and nurses to deliver high-quality care. Requires current PT licensure in state, valid driver's license, and CPR certification (preferred to complete within 90 days).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance/access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire (preferred)

Responsibilities

• Evaluates patient functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice and plan of care with health care team.
• Observes and reports patient responses to treatment and changes in condition to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient, family, and care team regarding plan of care, limitations, and progress.
• Visits patients per Plan of Treatment; completes progress notes.
• Completes all patient evaluations and develops PT plan within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
• Function in diverse home settings regardless of demographics.
