Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to perform assessments, develop and implement plans of care, and provide one-on-one care in patients\' homes within the local coverage area. The role emphasizes autonomy, flexible schedules, and meeting patient mobility and functional goals while collaborating with the health care team.

The position requires valid PT licensure in the practicing state and a valid driver's license.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates patient functional status and PT needs; assists in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment to supervising nurse/physician.
• Treats patients to relieve pain and restore function; maintains maximum functioning.
• Instructs patients and family/caregivers about plan of care and progress.
• Visits patients per Plan of Treatment; completes progress notes per policy.
