Job summary

Optum is seeking a Physical Therapist for Home Health to perform patient evaluations, develop and implement treatment plans, and collaborate with the care team to improve mobility and function. The role offers full-time hours, flexible schedules, mileage reimbursement, and autonomy in home-based care across the Sullivan County area.

Ideal candidates hold a current Physical Therapy license, a valid driver’s license, and are prepared to travel to patient homes.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Driver’s license and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient functional status and therapy needs; assists physician in the plan of care.
• Provides services within the scope of practice per plan of care and coordinates with health-care team.
• Observes and reports patient response to treatment and changes to supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family regarding plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Develops PT plan of care within state guidelines and reports outcomes.
