Job summary

UnitedHealth Group is seeking a Physical Therapist for home health in Sullivan County. The role includes evaluating patients, developing and implementing treatment plans, and coordinating with the health care team to improve mobility and function.

The position offers full-time flexibility, mileage reimbursement, and autonomy in providing one-on-one patient care in the home within the Sullivan County area.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs; assists physician in developing the plan of care.
• Provides services within the scope of practice per state laws and plan of care, in coordination with health care team.
• Observes and reports patient response to treatment and any condition changes to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning per standards.
• Instructs patient, family/caregivers regarding plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment; completes progress notes per policy.
• Completes patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
