Job summary

Qualifications

• Current Physical Therapy licensure in state of practice.
• Driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Works with the plan of care and other health care team members.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about plan of care, limitations, and progress.
• Visits patients according to Plan of Treatment and documents progress.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
