Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will evaluate functional status, create individualized plans with the care team, and supervise PTAs as needed, promoting independence and safe mobility outside the clinic.

Ideal candidates bring 1–2 years of home health experience, a current PT license, CPR certification, and strong communication with patients and caregivers to ensure quality, compassionate care across diverse

Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.

Responsibilities

• Evaluate patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and team.
• Provide hands-on therapy to improve mobility and independence at home.
• Educate patients and caregivers on exercises and safety.
• Document care and progress in the EMR and participate in team meetings.
