VitalCaring seeks a Home Health Physical Therapist to deliver patient-centered care in home settings. You will evaluate mobility, design and implement plans, provide hands-on therapy, and educate patients and families to restore independence.

The role emphasizes autonomy, teamwork with nurses and physicians, and ongoing clinical education to advance patient outcomes and your professional growth.

Qualifications

• Graduate from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the practiced state.
• Minimum 1–2 years of experience; home health preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification and valid driver’s license with reliable transport.

Responsabilités

• Evaluate patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor progress and adjust treatment plans; contribute to discharge planning.
• Supervise PTAs per state guidelines and support caregivers.
