Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to join a compassionate team delivering in-home care that helps patients regain mobility and independence. You will assess, plan, and implement individualized care while educating patients and families.

The role requires a PT license and 1–2 years of experience (home health preferred), CPR, and a valid driver’s license. You’ll collaborate with nurses, therapists, and physicians in a supportive, autonomous setting.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.

Responsibilities

• Deliver patient-centered home health care.
• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient care and progress accurately in the EMR.
• Supervise Physical Therapist Assistants (PTAs) per state guidelines.
