Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver autonomous, patient-centered care in patients' homes. You will assess mobility, develop individualized plans, and provide hands-on therapy to improve independence and quality of life.

The role emphasizes collaboration with nurses, physicians, and caregivers, with responsibilities including progress monitoring, education, and documentation in the EMR. A valid PT license and home health experience are preferred.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient care and progress accurately in the EMR; participate in team meetings.
