Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will assess function, mobility, and rehab needs, develop individualized plans with physicians, and help patients regain independence.

Qualifications

Graduate of an accredited Physical Therapy program (DPT or Master’s).
Current PT license in the state of practice.
Minimum 1–2 years of home health experience preferred.
Strong clinical assessment and critical thinking skills.
Excellent communication and patient education abilities.
Current CPR certification.

Responsibilities

Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
Develop and implement individualized care plans in collaboration with physicians and the care team.
Provide hands-on therapy to improve mobility, strength, balance, and independence.
Educate patients and caregivers on treatment plans, exercises, and safety strategies.
Monitor patient progress and adjust treatment plans as needed.
Support safe transitions of care and contribute to discharge planning.
Identify additional patient needs and collaborate with interdisciplinary teams.
Supervise Physical Therapist Assistants (PTAs) per state guidelines.
Document patient care and progress accurately within the EMR.
