Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in patients’ homes across Melbourne, FL. You will evaluate functional status, design plans with physicians, and provide hands-on therapy to restore mobility and independence.

The role emphasizes autonomy, collaboration with the care team, and ongoing education for patients and families, with opportunities for professional growth within a compassionate home health setting.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
