VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in the home setting. You will evaluate mobility, develop individualized plans, and provide hands-on therapy to improve independence and function.

This role emphasizes autonomy, collaboration with the care team, and ongoing education for patients and caregivers. Ideal candidates have a PT license in the state and 1–2 years of home health experience.

Qualifications

• Graduation from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsabilités

• Evaluate and assess patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and discharge planning.
• Document patient care and progress accurately within the EMR.
• Supervise PTAs in accordance with state guidelines.
• Lead and educate caregivers and support staff.
