Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to help patients recover mobility in their homes. You’ll work autonomously within the care team to deliver patient-centered therapy and support independence.

Requirements include a DPT or Master’s in Physical Therapy, an active state license, 1–2 years of home health preferred, CPR certification, and a valid driver’s license. We offer comprehensive benefits and flexible scheduling.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation and auto insurance

Responsibilities

• Evaluate patients’ functional status, mobility, and rehabilitation potential
• Develop and implement individualized home health care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance and independence
• Educate patients and caregivers on treatment plans, exercises and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Coordinate with nurses, therapists and physicians for coordinated care
• Document patient care and progress in the EMR
• Supervise PTAs and mentor staff
