VitalCaring Group is seeking a dedicated Home Health Physical Therapist to help patients regain independence in their homes. You will evaluate functional status, develop individualized plans, and deliver hands-on therapy focused on mobility, strength, balance, and safety.

You bring a DPT or Master’s in Physical Therapy, current PT license in state of practice, 1–2 years of experience (home health preferred), CPR certification, and a valid driver's license.

Qualifications

• Evaluate patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence
