Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes, helping them regain mobility and independence. You will lead care plans, collaborate with physicians and the care team, and educate patients and caregivers.

The role offers autonomy in a field-based setting with strong interdisciplinary collaboration. Qualified candidates have a DPT or Master’s in Physical Therapy, state licensure, CPR, and 1–2 years of home health experience.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of home health experience preferred
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately within the EMR
• Participate in case conferences and team meetings
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff
