21st Century Home Health Services (21HHS) is seeking a Full Time Occupational Therapist to serve Discovery Bay and surrounding communities. You will evaluate and address home health needs, enabling patients to live safely at home, work with an interdisciplinary team, and educate families.

Flexible schedules with autonomy, no clocking in/out, and strong benefits including 401k with matching, generous PTO, and comprehensive health coverage are offered.

Competenze

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Knowledge and understanding of state and federal homecare regulatory guidelines preferred.
• Experience in home health care is highly preferred.
• Experience with high acuitiy patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Self-Directed.
• Empathetic.
• Critical thinker.
• Outstanding interpersonal skills.
• Leadership skills.
• Organized.
• Pre-scheduling in-person interview requires completing a computer literacy assessment.

Mansioni

• Provide skilled OT services to evaluate and address home-based needs so clients can live safely and productively at home.
• Address home safety, medication management, wheelchair seating, positioning, pain management, falls prevention, chronic disease and dementia caregiver training.
• Serve as educator, patient advocate and member of the interdisciplinary care team.
