Optimal Care is seeking Nurse Practitioners or Physician Assistants to provide primary and consultative care for geriatric patients, often in home or care settings. The role emphasizes autonomy, high-quality patient-centered care, and collaboration with physicians, caregivers, and an interdisciplinary team.

Qualifications

• Master's or doctoral degree as NP/PA is required.
• Current State License as NP/PA is required.
• Reliable transportation, valid driver's license, and valid automobile insurance coverage.

Responsibilities

• Conduct comprehensive medical evaluations for geriatric patients.
• Develop and manage individualized treatment plans.
• Prescribe and monitor medications; coordinate with physicians, caregivers and the interdisciplinary team.
• Work within a collaborative practice framework as a key clinical decision-maker.
• Help patients and families understand diagnoses, manage chronic illness and avoid unnecessary hospitalizations.
• Coordinate with home health teams to ensure smooth patient transitions.
