Qualifications

• Valid NP or PA license with current board certification.
• Master's degree in nursing or physician assistant studies required.

Responsibilities

• Deliver comprehensive primary care in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with emphasis on prevention.
• Maintain EMR documentation and ensure regulatory compliance.
• Build trusted patient relationships and coordinate care with physicians and teams.
• Educate residents and families on disease management and wellness strategies.
• Collaborate with care teams to create comprehensive care plans.
