Job summary

Optimal Care seeks a Nurse Practitioner or Physician Assistant to provide primary and consultative geriatric care, often in patients' homes or care settings, with autonomy to focus on high-quality, patient-centered care that improves comfort and function.

You will perform comprehensive evaluations, develop treatment plans, prescribe medications, and collaborate with physicians and the care team to avoid hospitalizations.

Qualifications

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation and valid driver’s license.
