Job summary
BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for a full-time role to perform Start of Care admissions visits and comprehensive initial assessments for adult and geriatric clients in Monmouth, Middlesex, Union, and Somerset Counties, NJ.
You will develop plans of care, coordinate with an interdisciplinary team, educate clients and caregivers, document in the EMR, and travel throughout the territory while enjoying competitive pay and comprehensive benefits.
Qualifications
- Current and unrestricted New Jersey RN license.
- Minimum of one year of recent clinical nursing experience.
- Prior home health experience required.
- OASIS experience required.
- Strong clinical assessment and care coordination skills.
Responsibilities
- Complete OASIS SOC admissions visits for home health clients in the assigned territory.
- Perform initial nursing assessments and medication reconciliation.
- Develop individualized plans of care with physicians, clients, caregivers, and the team.
- Coordinate nursing, therapy, social work, and other services as needed.
- Educate clients and caregivers on diagnoses, medications, and home health plan of care.
- Document assessments and care coordination in the EMR.
