Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for a full-time role serving Monmouth, Middlesex, Union, and Somerset Counties, NJ. You’ll perform comprehensive SOC/ admission visits, assess clients, and craft individualized plans of care in collaboration with the interdisciplinary team.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.

Responsibilities

• Complete comprehensive OASIS Start of Care admissions visits for home health clients in the territory.
• Perform thorough initial nursing assessments, including medication reconciliation and safety evaluations.
• Develop individualized plans of care with physicians, clients, caregivers, and the team.
• Identify client needs and coordinate nursing, therapy, social work, and other services.
• Review physician orders and ensure the plan of care reflects client goals.
• Educate clients and caregivers on diagnoses, medications, and the home health plan.
• Communicate findings and changes in condition to physicians and BAYADA team.
• Ensure timely completion of OASIS assessments and admission docs.
• Document assessments and care coordination in BAYADA EMR.
• Support smooth transition from referral to admission and ongoing services.
