Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for full-time home health visits in North Jersey, covering Monmouth, Middlesex, Union, and Somerset Counties. You will conduct Start of Care assessments, develop individualized plans of care, and coordinate with physicians, clients, and caregivers to ensure seamless admission to BAYADA services.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.

Responsibilities

• Perform comprehensive Start of Care admissions visits.
• Develop individualized plans of care with the interdisciplinary team.
• Coordinate nursing, therapy, social work and home health services.
• Educate clients and caregivers about diagnoses and home health plan of care.
• Document assessments and care coordination in the EMR.
