Job summary

BAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for a full-time opportunity in New Jersey. The RN will perform Start of Care (SOC) visits and comprehensive assessments across Monmouth, Middlesex, Union, and Somerset Counties, coordinating care with an interdisciplinary team to ensure a smooth transition into home health services.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational and time-management skills.
• Ability to work independently and manage a field-based schedule.

Responsibilities

• Complete comprehensive OASIS SOC admissions visits for home health clients across the assigned territory.
• Perform thorough initial nursing assessments including med reconciliation and safety evaluations.
• Develop individualized plans of care with physicians, clients, caregivers, and the team.
• Identify client needs and coordinate nursing, therapy, social work, and other services.
• Review physician orders and ensure the care plan reflects client goals.
