Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Admissions for Home Health Visits in New Jersey. The role covers Monmouth, Middlesex, Union, and Somerset counties with a focus on Start of Care visits and comprehensive OASIS assessments.

You will educate clients and families, coordinate with the interdisciplinary team, and ensure accurate documentation in BAYADA's EMR. This is a full-time, field-based opportunity with collaborative support and growth opportunities.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.

Responsibilities

• Complete OASIS SOC admissions visits for home health clients in the assigned territory.
• Perform initial nursing assessments including medication reconciliation and safety evaluations.
• Develop individualized care plans with physicians, clients, and the interdisciplinary team.
• Coordinate nursing, therapy, social work, and other home health services as needed.
• Review physician orders and ensure plan of care reflects client needs and goals.
• Educate clients and caregivers about diagnoses, medications, and home health plan of care.
• Document assessments and care coordination in the EMR.
