Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for full-time home health admissions across Monmouth, Middlesex, Union, and Somerset Counties, NJ. You will perform Start of Care OASIS visits, conduct thorough assessments, and set up individualized care plans with physicians, clients, and families.

The role emphasizes autonomy, care coordination, and collaboration with an interdisciplinary team, delivering compassionate care to adults and seniors in their homes.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent clinical nursing experience.
• Prior home health experience is required.
• OASIS experience is required.

Responsibilities

• Complete OASIS SOC admissions visits for home health clients in the assigned territory.
• Perform comprehensive initial nursing assessments and medication reconciliation.
• Develop individualized plans of care with the interdisciplinary team.
• Identify client needs and coordinate nursing, therapy, social work, and other services.
• Review physician orders and ensure the plan reflects client needs and goals.
• Educate clients and caregivers about diagnoses, medications, and home health care.
• Document assessments, interventions, and care coordination in the EMR.
• Support smooth transition from referral to admission and ongoing services.
