Job summary

BAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for full-time home health admissions and SOC visits across Monmouth, Middlesex, Union, and Somerset Counties, NJ. You will assess, plan, and coordinate care for adult and geriatric clients within BAYADA’s interdisciplinary team.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Responsibilities

• Perform comprehensive OASIS Start of Care (SOC) admissions visits for home health clients in the assigned territory.
• Conduct thorough initial nursing assessments including medication reconciliation and safety evaluations.
• Develop individualized plans of care with physicians, clients, caregivers, and the interdisciplinary team.
• Coordinate nursing, therapy, social work and other home health services as needed.
• Review physician orders and ensure the plan of care reflects client needs and goals.
• Educate clients and caregivers about diagnoses, medications, treatments and home health care plans.
• Communicate findings and changes in condition to physicians and BAYADA team.
• Ensure timely completion of OASIS assessments and admission documentation.
• Document assessments, interventions, education and care coordination in the EMR.
