Job summary

FOX Rehabilitation in Katy, TX seeks a licensed Physical Therapist to deliver one-on-one, in-home care to older adults. You’ll treat patients in private residences or senior living communities, building mobility and independence through our Geriatric House Calls model.

You’ll enjoy autonomy over your schedule, flexible hours, and opportunities to incorporate telehealth when appropriate, while collaborating with PTs, OTs, SLPs, and fitness specialists to improve outcomes.

Qualifications

• Licensed Physical Therapist with current TX license or eligibility.
• Degree from an accredited Physical Therapy program (DPT).
• Basic computer and EMR proficiency; CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
