Job summary

FOX Rehabilitation in Hermitage, TN is seeking licensed Physical Therapists to provide one-on-one care in homes or Senior Living Communities. You’ll evaluate, create plans of care, and deliver evidence-based therapy that improves mobility and independence, with the flexibility to manage your own schedule.

Clinicians may incorporate telehealth when appropriate, collaborate with a multidisciplinary team, and educate patients and caregivers to promote aging in place.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop care plans, and deliver evidence-based treatment.
• Document in the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.

Education

Valid Physical Therapy license in the state(s) of practice

Degree from an accredited Physical Therapy program
