Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Camp Hill, PA, across private homes and senior living settings. You will assess mobility, create individualized plans, and help older adults improve independence while building lasting relationships.

The role offers autonomy, scheduling flexibility, and collaboration with PTs, OTs, SLPs, and Fitness Specialists, plus potential telehealth components and robust benefits for eligible clinicians.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults at home or in senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support mobility, independence, and safety
• Document effectively in the EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote aging in place and safety
• Provide telehealth/virtual care when appropriate to extend access
